葡萄球菌耐药性对尿路感染抗菌治疗影响因素分析

THE EFFECTS OF ANTIBIOTICS RESISTANT STAPHYLOCOCCUS ON URETHRAL INFECTION

目的了解近年来尿路感染患者中葡萄球菌的病原分布,分析其耐药性的变化对抗菌治疗的影响。方法收集2001—2006年尿路感染患者分离的葡萄球菌262株,用Kirby-Bauer法进行药敏测试,同时进行耐药检测。结果葡萄球菌在尿路感染检出率为12.21%,居第3位。以凝固酶阴性葡萄球菌为主(72.90%)。甲氧西林耐药金黄色葡萄球菌(methiciliin-resistant s.aureus,MRSA)和甲氧西林耐药凝固酶阴性葡萄球菌(methicillin-resistant coagulase-negative staphylococcus,MRCNS)发生率分别为50.70%、78.01%。万古霉素、替考拉宁敏感率均为100%。MRSA和MRCNS菌株对其它抗生素的敏感性多%30%。结论尿路感染患者分离的甲氧西林耐药葡萄球菌耐药严重,以凝固酶阴性葡萄球菌居多。宜选用万古霉素和替考拉宁。

Objective To reveal the distribution of staphylococcus for ,urethral infection in the region and the reason of antibiotic tolerance, and analyze the effects of antibiotics resistant stems of Staphylococcus on urethral infection. Methods To detect the 262 staphylococcus stems for urethral infection, and conduct the antibiotics sensitivity test according to Kirby-Bauer method. Results The presence of Staphylococcus was 12.21% in urethral infection, Staphylococcus was the third of the pathogens, among which, coagulase-negative Staphylococcus was 72.9 0%, methicillin-resistant S. aureus （MRSA） was 50.70% and methicillin-resistant coagulase-negative staphylocoecus（MRCNS） was 78.01%. The ratio of sensitivity to Vancomycin and Teicoplanin was 100%, the sensitivity to other antibiotics was below 30%. Conclusion MRSA is the main antibiotics resistant pathogen to the urethral infection,eoagulase-negative Staphylococcus is most often. The application of antibiotics should rely on the sensitivity of pathogenic test. Vancomycin and Teicoplanin are sensitive antibiotics to MRSA.
