强直性脊柱炎的脊柱外表现

EXTRASPINAL MANIFESTATION OF ANKYLOSING SPONDYLITIS

目的：提高对强直性脊柱炎脊柱外表现的认识。方法：回顾性分析了５４例住院确诊强直性脊柱炎的脊柱外表现。结果：强直性脊柱炎除了脊柱病变外，还可有外周关节、心、眼、肾、神经肌肉及全身表现，部分脊柱外表现如肌腱端炎、前葡萄膜炎、主动脉瓣关闭不全、心脏传导阻滞等可作为ＡＳ的早期诊断线索。以外周关节表现为主是ＡＳ误诊的主要原因。肾脏、肺、髋关节受累可能提示预后不良。

Objective:To enhance the understanding of the extraspinal manifestation of ankylosing spondylitis (AS).Methods:Fifty four AS in patients were analyzed retrospectively.Results:AS has several extraspinal manifestations including peripheral joints,eye,heart,kidney,nervous system,muscles involved,and fever and anemia.Enthesopathy,acute anterior uveitis,aortic incompetence and heart block are clues for early diagnosis of AS.Prominent manifestation of peripheral joints is the main cause for misdiagnosis of AS.The damage of kidney,lung and hip joint may indicate bad prognosis.Conclusion:The extraspinal manifestations of ankylosing spondylitis should be emphasized in clinical practice.
