内镜下注射治疗非食道静脉曲张出血的临床研究

CLINICAL INVESTIGATION OF ENDOSCOPIC INJECTION FOR NONVARICEAL UPPER GASTROINTESTINAL HEMORRHAGE

为了观察内镜下注射治疗对非食道静脉出血的疗效，该文对１９９４年１月～１９９８年１月用５％鱼肝油酸钠硬化治疗过的非食道静脉曲张出血病人４２例进行了研究。其中３２例为溃疡病，２例为复合溃疡或多发溃疡，５例为急性胃粘膜病变，１例为贲门粘膜撕裂，１例为Ｄｉｅｕｌａｆｏｙ病，１例为胃癌。出血方式：喷血或搏动出血３例（７．１％），渗血３７例（８８．１％），暂无出血的血管断端和血凝块４例（９．５％）。在局部注射５％鱼肝油酸钠硬化治疗后，３９例病人出血停止（占９２．９％），即刻止血率为７３．８％（３１例），３ｄ止血率为８１．０％（３４例）。硬化后未见到明显并发症。结论：内镜下注射治疗非食道静脉曲张出血安全、有效。

In order to investigate the effects of endoscopic injection for nonvariceal upper gastrointestinal hemorrhage,We studied 42 paients who had been treated with local injection of 5% sodium morrhuate under endoscopy from January 1994 to Januray 1998.There were 32 cases of peptic ulcer,two of coexistent ulcers or multiple ulcers,five of acute gastric mucosal erosino,one of mallory-weiss syndrome ,one of Dieulafory disease,one of gastric carcinoma.The severity of hemorrhage was as follows:spurting or pulsation,three cases (7.1%);oozing,37 cases (88.1%);nonbleeding visible vessel and clot,four(9.5%).After local injection of 5% sodium morrhuate,the hemorrhage in 39 case (92.9%)was stopped.Hemostatic rate immediately was 73.8%(31 cass )and 81.0%(34 cases) in 72 hours. No obvious complications was found after the injection .This results suggest endoscopic injection for nonvariceal upper gastrointestinal hemorrhage was effective and safe.
