改良筋膜内子宫切除术临床观察

Clinical observation of modified intra fascial hysterectomy

目的:观察子宫良性疾病需行子宫切除术的患者应用改良筋膜内子宫切除术、传统子宫切除术的治疗效果及并发症。方法:将106例因子宫良性疾病需行子宫切除的患者分为两组:观察组66例行经腹筋膜内子宫切除术,对照组40例行传统的经腹子宫全切术比较两组术中情况、疗效及并发症。结果:两组平均手术时间及术中出血量比较差异无显著性,下床活动时间、术后排气时间、阴道流血、对性生活满意程度及残端息肉的发生率等比较差异有显著性,观察组阴道流血发生率为0,性生活满意度89.3%、残端息肉发生率为0;对照组阴道流血发生率为15%,性生活满意度为65%,残端息肉发生率为7.5%。结论:改良筋膜内子宫切除术式对组织损伤小,保留盆底支持结构组织及阴道的完整性,手术效果好,有助于提高患者的生活质量,优于传统子宫切除术。

Objective：To observe the curative effect of modified intra fascial hysterectomy and traditional hysterectomy in uterine benign diseases needing hysterectomy and its complications. Methods：The patients with uterine benign disease needing hysterectomy were divided into two groups.66 cases in study group were performed the modified intra fascial hysterectomy, while 40 cases in the control group were performed the traditional transabdominal hysterectomy.The operation condition, curative effect and complications were compared.Results：The mean operative time and bleeding volume had no significant difference,and get out of bed time,length of hospital stay, postoperative discharge time, recovery time and life satisfaction of sexual life, the incidence of stump polyps,ere.had significant difference.In the study group,the sexual satisfaction was 89.3%, the incidence of stump polyps 0;in the control group, the sexual satisfaction was 65%, the incidence of stump polyps was 7.5%.Conclusion ：The modified intra fascial hysterectom has minimal tissue damage,retains the pelvic support structures and vaginal integrity with good curutive effect.It is helpful to improve the quality of life of patients and superior to the traditional hysterectomy.
