慢性阻塞性肺病合并呼吸衰竭患者长期氧疗配合中药汤剂治疗的研究

Long-term Oxygen Therapy in Concert With Traditional Chinese Medicine(TCM) Decoction in Patients With Chronic Obstructive Pulmonary Disease(COPD) and Chronic Respiratory Failure

目的:研究慢性阻塞性肺病(Chronic obstructire pulmonary disease,COPD)并慢性呼吸衰竭患者应用长期氧疗配合中药汤剂的疗效。方法:慢性阻塞性肺病并慢性呼吸衰竭患者使用长期氖疗,与长期氧疗配合中药汤剂患者比较在治疗的1年中的急性加重次数、住院次数、住院时间、住院费用、圣.乔治呼吸量表评分、以及营养学指标(%IBW、TP、ALB、PALB、)、肺功能参数(FVC、FEVl.0%、MIP)、血气分析指标(pH、PaO2、PaCO2)的变化。结果:慢性阻塞性肺病并慢性呼吸衰竭患者应用长期氧疗配合中药汤剂后,总蛋白、白蛋白、前白蛋白升高,PaCO2、和圣.乔治呼吸评分降低,pH、PaO2增加,肺功能参数改善,与单纯氧疗组相比,长期氧疗配合中药汤剂组急性加重次数、住院次数、住院费用减少。结论:长期氧疗配合中药汤剂可以很好的改善营养状态,改善呼吸困难、缓解呼吸肌疲劳,延缓肺功能的进行性下降,减少急性加重次数、住院次数及住院费用,是治疗慢性阻塞性肺病并慢性呼吸衰竭积极和有效的治疗措施。

Objective： To ascertain the clinical effect of Long - term oxygen therapy in concert with traditional Chinese medicine（TCM） decoction in patients with chronic obstructive pulmonary disease（COPD） and chronic respiratory failure. Methods ： Test group COPD patients complicated by chronic respiratory failure received Long - term oxygen therapy in concert with traditional Chinese medicine（TCM） decoction. The frequency of acute exacerbation, hospitalization times, hospitalization expense. St · George＇ s respiratory questionnaire score, blood gas analysis, and pulmonary function, Nutritional status results were compared in the test group with the control group patients those who received long term oxygen therapy. Results： After one year treatment,TP,ALB,PALB of test group were increased,PaCO2 and St. George＇ s respiratory questionnaire score were greatly ameliorated, Compared with control group the frequency of acute exacerbation, admission times and admission expense declined. Conclusion： Long - term oxygen therapy in concert with traditional Chinese medi- cine （TCM） decoction can improve the nutritional status, relieve dyspnea and alleviate respiratory muscle fatigue, delay the progressive decline of 1 ung function, reduce the acture exacerbation frequency, hospitalization frequency and hospitalization costs. Long - term oxygen therapy is a positive and effective treatment against the chronic obstructive pulmonary disease and chronic respjratory failure.
