摘要
目的:探讨胃癌组织胸苷磷酸化酶(TP)和缺氧诱导因子-1α(HIF-1α)的表达及与预后的相关性。方法:选取胃癌标本62例及20例正常胃黏膜组织,进行免疫组化检测TP、HIF-1α表达,并将结果与患者术后生存时间进行比较。结果:TP及HIF-1α在62例胃癌组织的阳性表达率分别为69.00%和66.00%。TP表达与肿瘤大小(χ2=8.988,P=0.030)、浸润深度(χ2=8.240,P=0.040)、淋巴结转移(χ2=13.964,P=0.000)、远处转移(P=0.047)和PTNM分期(χ2=6.066,P=0.014)相关。HIF-1α表达则只与肿瘤浸润深度(χ2=5.885,P=0.015)、淋巴结转移(χ2=4.353,P=0.037)和PTNM分期(P=0.029)相关。TP与HIF-1α的表达不相关,rs=0.205,P=0.110。TP、HIF-1α阳性者5年生存率明显低于阴性者,P<0.05。多因素分析表明,淋巴结转移、远处转移、HIF-1α的表达及PTNM分期是胃癌患者独立预后因子。结论:TP、HIF-1α在胃癌组织中高表达,但两者不相关。TP、HIF-1α过表达与胃癌的不良生物学行为有关,可能成为预测胃癌侵袭、发展及判断预后的重要指标。
OBJECTIVE: To explore the expressions of thymi dine phosphorylase (TP) and hypoxia inducible factor -1α(HIF-1α) in gastric carcinoma, and analyze their correlations with clinieopatholog ic features and prognosis. METHODS: The expressions of TP and HIF-1α in 62 cases of gastric carcinoma specimens and 20 normal gas tric tissues were examined by the immunohistochemical method. The postoperative survival rate was observed in these patients. RESULTS: The positive rates of the expressions of TP and HIF-1α protein were 69.00 % and 65.00 % in 62 patients with gastric carcinoma respective ly. TP expression was positively related to the tumor size (x^2 = 8. 988,P=0. 030), depth of invasion(x^2= 8. 240,P= 0. 040), lymph node metastasis (x^2 13. 964,P=0.000), distant metastasis (P= 0.047), and clinical stage (x^=6.066,P=0.014). HIF-1α was only related to the depth of tumor invasion (x^2 = 5. 885, P= 0.015), lymph node metastasis (x^2=4.353,P =0.037), and clinical stage (P=0. 029). There was no relationship between the expressions of TP and HIF-1α (rs =0. 205, P=0. 110). Moreover, the 5 year survival rate was significantly better in the TP/ HIF-1α-negative group than that in the TP/HIF-1α/ positive group (P〈0.05). Multivariate analysis showed that lymph node metastasis, distant metastasis, and the expressions of HIF-1α and PTMN stage were independent prog nostic factors in patients with gastric carcinoma. CONCLUSIONS: TP and HIF-1α are overexpressed in gastric carcinoma, but there is no correlation between TP expression and HIF la expression. The over expressions of TP and HIF-1α are related to the poor biological behav ior and prognosis of gastric carcinoma.
