掌长肌腱腱片移植治疗陈旧性锤状指畸形

Treatment of chronic mallet finger deformity by pahnaris longus tendon graft

目的探讨掌长肌腱腱片移植治疗陈旧性锤状指畸形的疗效。方法对28例陈旧性锤状指畸形的患者，采用克氏针固定远指间关节、掌长肌腱腱片移植加强修复伸肌止点的手术方法。术后6周拔出克氏针，随访时按照Patel评价体系评定。结果术后25例获得随访，3例失访，随访时间为3-15个月，平均10个月。优9例，良13例，可2例，差1例；优良率为88％。结论采用掌长肌腱腱片移植加强修复指伸肌腱断端，可明显纠正畸形，并获得良好的关节活动度，是治疗陈旧性锤状指畸形较有效的方法。

Objective To evaluate the clinical outcomes of treating chronic mallet finger deformity using palmaris longus tendon graft. Methods Twenty-eight cases of chronic mallet finger deformity were treated by palmaris longus tendon graft to strengthen and repair the affected extensor tendon. The distal interphalangeal joint was fixed in extension with a K-wire, which was kept for 6 weeks before removal. The patients were followed up postoperatively and evaluated using the Patel criteria. Results Twenty-five cases were followed while 3 were lost to follow up. The follow up period ranged from 3 to 15 months, with an average of 10 months. The results were rated as exeellent in 9 eases, good in 13 cases, fair in 2 cases and poor in 1 case. The overall satisfactory rate was 88%. Conclusion Using palmaris longus tendon graft to strengthen the repaired extensor tendon can correct the deformity and obtain good motion of the distal interphalangeal joint. It is an effective treatment method for chronic mallet finger deformity.
