溃疡性结肠炎患者血浆P选择素水平及血小板计数的变化及其临床意义

Clinical significance and diversification of plasma P-selectin and platelet count in patients with ulcerative colitis

目的探讨溃疡性结肠炎（UC）患者血浆P选择素（P—sel）水平及血小板计数的变化及其临床意义。方法采用酶联免疫分析方法检测45例UC患者和20例同期健康体检者血浆P—sel水平，采用血细胞记数仪测定血小板计数。结果随着病情的加重，UC患者血浆P-sel水平和血小板计数逐渐升高，差异有统计学意义（P〈0．01）。同时，UC活动期血浆P-sel水平[（31．9±6．9）μg/L]明显高于对照组[（15．6±4．8）μg/L]和uc缓解期[（22．1±5．5）μg/L]（P〈0．01）；UC缓解期明显高于对照组（P〈0．05）。UC活动期血小板计数[（230．8±75．9）×10^9／L]明显高于对照组[（163．1±47．8）×10^9／L]（P〈0．01），明显高于UC缓解期[（167．3±49．5）×10^9/L]（P〈0．01）；UC缓解期与对照组比较，差异无统计学意义（P〉0．05）。结论P—sel与UC的发病和病情进展密切相关，临床联合检测UC患者血浆P—sel水平及血小板计数，对病情的判断及其疗效的评估有一定参考价值。

Objective To study the clinical significance and diversification of plasma P-selectin and platelet count in patients with ulcerative colitis （ UC ）. Methods The levels of plasma P-selectin were detected by enzyme-linked immunosorbnent assay; the platelet count was detected by blood cell counting instrument in patients with UC （n = 45 ） and healthy people recieving medical examination during the same period （n = 20 ）. Results With the progress of UC, the plasma P-selectin and platelet count in patients with UC gradually increased （ P 〈 0.01, P 〈 0.05 ）. The levels of plasma P-selectin in the active UC group were significantly higher than those in the control group （P 〈0.01 ）, significantly higher than those in the remittent UC group（P 〈0.01 ）. The platelet count in the active UC group were significantly higher than that in the control group （ P 〈 0.01 ） and significantly higher than that in the remittent UC group （P 〈 0.01 ）. Conclusion P-selectin is closely related to the incidence of UC and the progress of disease. Combined detection of plasma P-selectin and platelet count in patients with UC has reference value for the disease estimation and the efficacy assessment.
