摘要
目的通过测定血浆及支气管肺泡灌洗液(BALF)IL-8浓度等参数探讨高碳酸血症对兔急性呼吸窘迫综合征的保护作用。方法通过油酸静脉注射建立兔急性呼吸窘迫综合征动物模型并行气管切开连接小型动物呼吸机(呼吸模式为AC模式,潮气量为8 mL/kg;呼吸频率为51次/min;PEEP为5 cm H2O;吸入氧浓度FiO2为0.60)。将造模成功兔随机分为两组,实验组给予外源性CO2吸入,吸入浓度(FiCO2)为0.06,实验对照组不给予外源性CO2吸入。分别观察试验组与实验对照组兔于造模成功后180 min血浆及支气管肺泡灌洗液(BALF)中白细胞介素-8(IL-8)水平。结果试验组兔于造模成功后180 min血浆及支气管肺泡灌洗液(BALF)中白细胞介素-8(IL-8)水平明显低于对照组。结论高碳酸血症能够降低兔急性呼吸窘迫综合征肺局部和血液IL-8水平,进而减轻肺局部和全身炎症损伤,对兔急性呼吸窘迫综合征产生保护作用。
Objective To investigate the effect of hypercapnia on rabbits with acute respiratory distress syndrome through detected serum IL-8 and BALF IL-8 level.Methods Establish rabbit model with acute respiratory distress syndrome induced by oleic acid.Connected with animal ventilator and were mechanically ventilated after making tracheotomy(ventilator parameter,VT 8ml/kg;R 51/min;PEEP 5cmHO2;FiO2 0.60).Rabbits were allocated randomly into two groups,experiment group was given CO2(FiCO2 0.06);control group wasn't given.Respectively compare with serum IL-8 level,BALF IL-8 level between two groups after 180 minutes.Results The level of serum IL-8 and BALF IL-8 in the experiment group is lower than the control group after ventilated mechanically for 180 minutes.Conclusion Hypercapnia can decrease BALF IL-8 level and serum IL-8 level,and have protective effects to rabbits with acute respiratory distress syndrome through alleviating the partial and systemic inflammatory injury.
