经皮内镜下胃造瘘术在危重患者肠内营养中的应用及护理 被引量:4

摘要 目的探讨经皮内镜下胃造瘘术在危重患者肠内营养中的应用价值及护理要点。方法通过对2002年1月-2009年5月18例患者进行胃造瘘术(PEG),建立肠内营养支持及开展相应的护理。结果所有患者一次置管成功(成功率100%),术后有3例因原发病加重在1个月内死亡,15例在原发病控制和抗感染等综合治疗下带造瘘管转出重症监护室。患者术中血压稳定,手术操作时间20.5±2.5min,经再次胃镜检查造瘘管位置良好。所有患者术后观察1—5个月,穿刺部位术后无出血。2例患者术后切口发红,经局部换药后好转。其余患者未观察到出血、意外拔管、腹膜炎、胃结肠瘘、反流误吸、吸入性肺炎。结论PEG是一种操作方便,快捷,安全度高,术后并发症少,留置时间长的治疗技术,通过高质量的护理,能给予良好的营养支持,值得推广。
