关节镜下治疗膝关节盘状半月板损伤手术35例

Treatment of Discoid Meniscus Injury Under Arthroscopy

目的探讨关节镜下治疗盘状半月板损伤的疗效。方法对35例盘状半月板损伤患者行关节镜下手术治疗，其中不完全型14例，完全型21例。根据术中情况分别予以部分切除成形、次全切除乖全切除。结果平均随访1-4年，按Ikeuehi标准，优20例，良11例，优良率88.6％。完全型盘状半月板中，全切优良率为86.7％，部分切除成形术优良率为50％；不完全型盘状半月板中，全切优良率为75％，部分切除成形术优良率为83.3％。结论关节镜下治疗盘状半月板行半月板成形术可以获得良好效果；完全型盘状半月板如伴有复合裂损伤主张行盘状半月板完全切除术，而不全型半月板主张尽可能行关节镜下盘状半月板部分切除成形术。

Objective To investigate the effect of treatment of discoid meniscus injury under arthroscope. Methods 35 cases of discoid meniscus injury were treated under arthroscope,21 knees were complete injury and 14 incomplete. 35 ease of discoid meniscus injury were treated by arthoscopy：part excision, reshaping, subtotal excision and total excision respectively according to conditions of operations. Results All the cases were followed up for an average of 3 years. Based on Ikeuchis grading,20 cases excellent, 11 good, the excellent and good was 88.6%. In cases of complete type of discoid meniscus, the excellent and good result was 86.7% which patients treated with total meni scectomy ;the excellent and good result was 50.0% which patients treated with partial meniscectomy; the difference was significant. In cases of incom- plete type of discoid meniscus, there were no significant difference between total and partial meni scectomy. Conclusions Arthroscopic management for discold meniscus can obtain excellent and good efficacy. Patients suffered complete type of di SCOid meni SCUS should be treated with total meni scectomy. While incomplete ones should be treated with partial meni sccctomy.
