摘要
目的探讨腹型肥胖指标腰围身高比值(WHtR)与2型糖尿病患者颈动脉内膜中膜厚度(IMT)的关系。方法117例住院2型糖尿病患者,以WHtR0.50为切点分为A组(WHtR〈0.50)42例和B组(WHtR≥0.50)75例,分析wHtR与IMT的关系。结果B组体质量指数(BMI)、腰围、胰岛素抵抗指数(HOMA—IR)和IMT均高于A组,BMI 28.41±2.40 vs 21.20±2.60、腰围(95.92±8.71)mm vs (74.91±6.30)mm、HOMA—IR 1.67±0.65 vs 0.85±0.70、IMT(0.89±0.10)mm vs(0.81±0.14)mm。多元线性回归分析显示WHtR是IMT的独立危险因素[偏回归系数(8)=0.072,t=4.808,P=0.000]。logistic回归分析显示wHtR≥0.5者发生IMT增厚的危险是WHtR〈0.5者的3.720倍(95%CI=1.391~9.948,Wold X^2值=6.854,P=0.009)。结论wHtR与IMT密切相关,可作为预测IMT增厚的简便身体测量指标。
Objective To evaluate the relationship between waist to height ratio(WHtR) and carotid intimamedia thickness(IMT) in type 2 diabetes patients. Methods The study included 117 diabetes in-patients, who were divided into two groups: A group(WHtR〈0.50, n = 42) and B group (WHtR≥0.50, n = 75). The relationship between WHtR and IMT was analyzed. Results B group had significantly higher levels of body mass index (BMI), waist circumference, HOMA-IR and IMT than A group( P 〈0.05), BMI 28.41±2.40 vs 21.20±2.60, waist (95.92±8.71) mm vs (74.91±6.30) mm, HOMA-IR 1.67±0.65 vs 0.85±0.70 and IMT (0.89±0.10) mm vs (0.81±0.14) mm. Multiple linear regression analysis showed that WHtR was a significant independent predictor for IMT(β= 0. 072, t = 4. 808, P=0. 000). Odds rations of the thickening of carotid artery IMT in B group was 3. 720 times(95% CI=1. 391-9. 948, P =0.009) against A group. Conclusion IMT is associated with WHtR and can serve as a measure of abdominal fat accumulation,WHtR may be a useful predictor of IMT.
