摘要
目的观察老年恶性心包积液患者心包腔内分别灌注香菇多糖与顺铂治疗的有效性和安全性。方法45例患者随机分为A组22例,B组23例,采用中心静脉穿刺导管行心包穿刺置管引流术,A组给予灌注香菇多糖4 mg+生理盐水20 ml,B组给予顺铂40 mg+生理盐水20 ml。结果A组患者有效率(完全缓解率+部分缓解率)90.9%(20/22)例,B组患者有效率60.9%(14/23)例,A组患者有效率优于B组(P<0.05),且A组不良反应轻微。结论对于老年恶性心包积液的患者心包腔内灌注香菇多糖有效而且安全。
Objective To observe clinical therapeutic effectiveness and adverse reaction in the treatment of elderly pationts with malignant pericardial effusion by injection to pericardial cavity with lentinan or cisplatin alone. Methods Forty-five patients with malignant pericardial effusion were divided into two groups：in group A （22 cases）, lentinan was used,in group B （23 cases） cisplatin was injected. One-off central venous catheter was used to effuse the malignant pericardial effusion in group A （lentinan 4 mg＋NS 20 ml） and group B （cisplatin 40 mg＋NS 20 ml）. Results The overall objective responded rate （complete and partial remission） of group A was preponderated over group B, 90.9 （20/22） vs 60. 9%）（14/23）（ P 〈0.05）. No severe side effects were observed in group A. Conclusion The perfusion of lentinan has better effect than cisplatin for the treatment of malignant pericardial effusion in elderly patients with mild side effects.
