新鲜与陈旧性股骨颈骨折全髋关节置换术疗效对比分析

Comparative analysis of total hip replacement for the treatment of fresh and old femoral neck fractures

目的比较全髋关节置换术治疗新鲜与陈旧性股骨颈骨折的疗效。方法对2005年1月至2007年12月在本科行全髋关节置换术的股骨颈骨折患者进行回顾性分析,其中新鲜股骨颈骨折21例,陈旧性股骨颈骨折22例。主要比较指标:手术时间、术中出血量、引流量、术后下地时间、并发症等,髋关节功能评定按Harris评分标准,手术疗效按功能改善率进行评定。结果陈旧性股骨颈骨折组手术时间、出血时间及引流量高于新鲜股骨颈骨折组,陈旧性股骨颈骨折组术前Harris评分高于新鲜股骨颈骨折组,术后两组Harris评分无显著区别,两组手术疗效优良率均为100%,两组病例未出现假体脱位、骨折再发等并发症。结论全髋关节置换术对股骨颈骨折疗效良好,陈旧性股骨颈骨折与新鲜骨折术后关节功能评分无显著区别。

Objective To compare the effects of total hip replacement in the treatment of fresh and old femoral neck fractures. Methods Femoral neck fracture patients treated with total hip replacement from January 2005 to December 2007 were analyzed retrospectively,of which 21 cases were fresh femoral neck fracture and 22 cases were old femoral neck fracture. The main comparative indicators were as follows .. opevation time, bleeding volume, drainage volume, postoperative time to get out of bed, complications, etc. Hip functions were evaluated by Harris standards. Therapeutic effects were assessed according to the improvement rate of hip functions. Results Opevation time,bleeding time and drainage volume of the old femoral neck fracture group were higher than the fresh femoral neck fracture group. Harris scores preoperative in the old femoral neck fracture group were higher than the fresh femoral neck fracture group. Harris scores after operation of the two groups had no significant difference. Excellent and good rates of two groups were both 100 percent. Complications such as prosthesis dislocation and fracture did not occurred. Conclusion Total hip replacement is efficient for femoral neck fracture. There is no significant difference between joint function scores of the fresh and old femoral neck fractures.
