应激性高血糖及糖尿病患者糖化血红蛋白的临床研究 被引量:4

摘要 目的:探讨糖化血红蛋白(HbAlc)检测在应激性高血糖以及糖尿病患者中的临床价值及意义。方法:在伯乐BIO-RAD D-10糖化血红蛋白系统上使用高压液相层析法(HPLC)对我院住院患者中测得高血糖者以及糖尿病患者进行HbAlc测定,并追踪血糖状态最终诊断,对于HbAlc低于正常者加测多时间段血糖,进行临床分析。结果:糖尿病高血糖者HbAlc9.85±1.22%,应激性高血糖者HbAlc7.64±1.25%,糖尿病低血糖者HbAlc3.56±1.11%。经t检验,应激性高血糖者与糖尿病高血糖HbAlc有显著性差异(P<0.01)。结论:HbAlc可以作为应激性高血糖与糖尿病高血糖鉴别诊断的简单、有效的方法推广应用。HbAlc检测有利于发现夜间及无症状低血糖,有利于监测和诊断糖尿病持续、稳定、安全达标。 Objective: To explore the clinical significance of glycated hemoglobin in n-eating stress hyperglycemia and diabetes. Methods: The content of glycated hemoglobin of inpatients with diabetes and high blood glucose was determined by high-pressure liquid chroma- tography(HPLC) based on Bio-Rad D-10 glycated hemoglobin systems. And the final diagnosis was tracked. For cases with lower level of glycated hemoglobin, further determination was carried out. Results:The average content of glycated hemoglobin in patients as diabetes with high blood glucose was 9.85 ± 1.22. And for stress hyperglycemia, 7.64 ± 1.25 ; for diabetes with hypoglycemia, 3.56 ± 1.11. There had significant difference of glycated hemoglobin between group of stress hyperglycemia and that of diabetes with high blood glucose ( P 〈 0.01 ). Conclusion: The determination of glycated hemoglobin can be used as a simple, effective index of differential diagnosis between stress hyperglycemia and diabetes with high blood glucose.
