复张性肺水肿的病因及防治

The etiology and treatment of re - expansion pulmonary edema

目的探讨复张性肺水肿的病因及有效防治措施。方法分析16例复张性肺水肿临床资料，主要治疗包括气管内插管，呼吸机辅助，并改善血流动力学等。结果治愈15例，1例未上呼吸机，因心肺功能衰竭死亡。结论复张性肺水肿是一罕见的特定条件下发生的急性肺损伤，缓慢逐渐膨肺是有效的预防措施。其治疗具有较强的时效性和个体化性，强调早期呼吸机支持，早期纠正血流动力学紊乱。

Objective To explore the etiology, incidence conditions, effective prevention and treatment of re - expansion pulmonary edema（REPE）. Methods The clinical data of 16 cases of REPE were analyzed. Treatments included endotracheal intubation, mechanical ventilation, and improvement of hemodynamics. Results Fifteen cases were cured, and one case died of heart and lung failure. Conclusion REPE is a rare acute lung injury which occurs in certain conditions -- a rapidly re - expanding lung has been in a state of collapse for more than 7 days. An effective preventive measure is to expand the lung gradually and slowly. It should be performed to provide promptly early artificial lung ventilation and correct hemodynamic disorders after REPE occurs.
