摘要
目的探讨影响单纯性肥胖儿童体液免疫功能的相关因素。方法选择符合诊断标准的80例单纯性肥胖儿童(研究组)和80例健康儿童(健康对照组),测定两组的腹围、身高、体重,计算体质指数(BMI)。运用散射比浊法测定免疫球蛋白IgG、IgA、IgM及补体C3、C4,同时测定空腹血糖、血脂、胰岛素,两组进行比较并探讨影响体液免疫功能相关因素。结果研究组的IgG、IgM、IgA和补体C3浓度值均明显低于正常儿童,且研究组儿童的肥胖年数、腹围、BMI、FPG、甘油三酯(TG)、总胆固醇(TCH)、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HBCI)密切相关。经多因素回归分析,影响体液免疫功能的因素为腹围、BMI、TG、肥胖年数和HOMA-IR。结论单纯性肥胖症患儿存在体液免疫功能受损,其与腹围、肥胖体重指数和年数、血脂及胰岛素的抵抗有关。
Objective To explore the impact of obesity on children's humoral immunity by comparing the humoral immune function and analyzing the possible factors associated to humoral immunity. Methods Eighty children who met the criterion of simple obesity and 80 age and sex matched healthy children were included in this study. Abdomen circumference, height, and weight were measured, BMI was assessed. Three different immunoglobin subtypes (IgG, IgA, IgM), alexin 03, alexin 04, blood lipid, fasting plasma glucose, and fasting plasma insulin were examined. Results The IgG, IgM, igA and alexin 03 of simple obesity children were significantly lower than those of healthy children. Meanwhile, body mass index (BMI), FPG, TG, the years of obesity, abdominal circumference and the sensitivity indexes of insulin (HOMA- IR and HBCI) were closely associated with humnoral immune dysfunction. By multiple linear regression analysis, humnoral immune dysfunction (IgG) was significantly correlated with abdomen circumference, BMI, TG, the years of obesity, and HOMA- IR (P〈 0.05). Conclusions There is the dysfunction of humoral immunity in the simple obese children. Humoral immunity damage is related to the factors including BMI, abdomen circumference, the years of obesity, blood lipid and the sensitivity indexes of insulin.
