主动脉瘤影像学诊断评价

EVALUATION OF IMAGE DIAGNOSIS FOR AORTIC ANEURYSMS

目的：对主动脉瘤不同的影像学诊断方法准确性进行评价。方法：４７例主动脉瘤病人术前常规行Ｘ线平片、超声心动（ＵＣＧ）检查，选择性行主动脉造影、数字减影（ＤＳＡ）及核磁共振（ＭＲＩ）检查。检查结果与手术发现进行比较。结果：ＵＣＧ对升主动脉瘤、ＤｅＢａｋｅｙＩ、Ⅱ型夹层主动脉瘤诊断准确率较高，主动脉造影准确率低于ＤＳＡ及ＭＲＩ，ＤＳＡ及ＭＲＩ准确率相似，ＭＲＩ夹层阳性率高于ＤＳＡ。结论：常规ＵＣＧ检查结合选择性主动脉造影。

Objective:To Evaluate the diagnostic accuracies of different image methods for aortic aneurysms.Methods:X ray plane films and ultrasonic cardiography (UCG) were routinely ,retrograde aortography,digital subtract angiography(DSA) and magnetic resonance image(MRI) were selectively,performed before operation in 47 patients with aortic aneurysm.The image results were compared with surgical findings during operation.Results:UCG had a high accuracy for ascending aortic aneurysms,DeBakey Ⅰ and Ⅱ type dissecting aortic aneurysms.Accuracy of aortography was lower compared with DSA and MRI.Though DSA had a similar accuracy to MRI,the positive rate of dissections in MRI was higher than that in DSA.Conclusion:Routine UCG combined with aortography,DSA or MRI can provide enough informations meetin the needs of surgical policy making for aortic aneurysms.
