银杏叶提取物联合厄贝沙坦治疗早期糖尿病肾病的临床研究

Clinical study on treatment of early diabetic nephropathy by Extract of Gingko biloba combined with irbesartan

目的:观察银杏叶提取物联合厄贝沙坦对早期糖尿病肾病的作用效果并研究其可能的保护机制。方法:将61例早期DN患者随机分为对照组(30例)和治疗组(31例),对照组为常规西药治疗,治疗组在此基础上加用银杏叶提取物片,疗程均为6个月。采用酶联免疫吸附法(ELISA)测定两组患者治疗前后血清转化生长因子β1、Ⅳ型胶原变化水平,同时观察尿白蛋白排泄率、空腹血糖、血肌酐、尿素氮、血脂等指标的变化。结果:与治疗前比较,两组患者治疗后血清转化生长因子β1、Ⅳ型胶原水平下降(P<0.05),治疗组较对照组下降更为明显(P<0.05)。治疗组治疗后尿白蛋白排泄率明显降低,血脂水平改善(P<0.05)。结论:银杏叶提取物可通过降低早期糖尿病肾病患者血清转化生长因子β1、Ⅳ型胶原水平而延缓糖尿病肾病的发展。

Objective： To study the effects and the possible mechanism of Extract Gingko biloba combined with irbesartan on early diabetic nephropathy. Methods： Sixty-one patients with early diabetic nephropathy were randomly assigned into the control group （n = 30 ） treated by irbesartan alone and the treated group （n =31 ） treated by Extract Gingko biloba combined with irbesartan. The treatment course was 6 months. Transforming growth factor β1 and serum collagen Ⅳ were determined with ELISA before and after treatment. Urinary albumin excretion rate, fasting blood glucose, serum creatinine, blood urea nitrogen and plasma fat, etc. were examined as well. Results： The levels of serum transforming growth factor β1 and collagen Ⅳ were significantly lower in both groups after treatment than those before treatment （ P 〈 0.05 ）, and the decrement in the treatment group was more significant than that in the control group （P 〈 0.05）. And the levels of urinary albumin excretion rate, fasting blood glucose decreased in the treatment group after treatment （P 〈 0.05）. Conclusions： Extract Gingko biloba could retard the development of early diabetic nephropathy through decreasing the levels of transforming growth factor β1 and collagen Ⅳ.
