腓骨长肌腱移位修复陈旧性闭合性跟腱断裂的临床应用

Clinical application of peroneal muscles tendon transposition for the repair of old achilles tendon rupture

目的:探讨腓骨长肌腱移位修复陈旧性闭合性跟腱断裂的手术方法。方法:设计腓骨长肌腱移位重建跟腱的术式,并在临床应用于20例陈旧性闭合性跟腱断裂的患者,其中,男14例,女6例。跳跃伤12例,砸伤4例,踏空伤及自发伤各2例。均为陈旧伤,受伤至手术时间21天～3个月。全部患者均为闭合性跟腱断裂,其中完全性断裂4例,不完全性断裂16例。结果:临床应用20例患者切口均Ⅰ期愈合,均获随访18～24个月,无再断裂发生,无皮肤坏死等手术并发症。按照Amer-Lindholm评定标准:优14例,良6例,优良率100%,跟腱功能恢复良好。结论:腓骨长肌腱移位修复跟腱对于陈旧性闭合性跟腱断裂,是一种方法简便,疗效满意的手术方法。

Objective：To discuss surgical procedures of long peroneal muscles tendon transposition for the repair of occlusive achilles tendon rupture.Methods：10 cases of occlusive achilles tendon rupture were treated by use of long peroneal muscles tendon transposition from March 2001 to July 2008,Among seven men and three female,six cases of jump injure,two cases of bruise,one case of step vacancy and spontaneity injure,all cases of old injure lasted 21d-3months from Achilles tendon rupture to surgery, all cases belong to occlusive achilles tendon rupture in which two perfectibility rupture,sixteen imperfectability. Results： 20 cases of occlusive achilles tendon rupture were treated and the incision of surgery all closed up I grade. All cases were followed up 18 24 months,none of ones were found repair of achilles tendon,repture again or skin ecrose and found other operate complications. According to Amer lindholm criterion for curative results,the results were excellent in 16 cases and good in 4 cases, with the excellent and good rate being 100G. Conclusions： It was perfect and simple surgical procedure that long peroneal muscles tendon transposition for the repair of occlusive Achilles tendon rupture.
