山东省首例人禽流感病例的诊断过程分析

Diagnostic procedure analysis of the first human case of Highly Pathogenic Avian Influenza Virus (H5N1) in Shandong Province,China

目的对2009年1月15日山东省济南市发生的1例人禽流感病例进行实验室诊断过程分析,探讨实验检测过程中遇到的问题。方法采集病人咽拭子和呼吸道吸取物标本,利用RT-PCR和real-time RT-PCR检测病毒核酸。结果第2次采集的咽拭子标本采用H5HA引物的RT-PCR扩增呈阳性;呼吸道吸取物分别用H5NA等3对引物进行RT-PCR均阳性,real-time RT-PCR阳性。结论该患者为人感染高致病性禽流感病毒(H5N1)实验室确诊病例。呼吸道吸取物是禽流感病毒核酸检测的合适标本。

Objective To analyze the laboratory diagnosis of a human case of Avian Influenza Virus （H5N1） in Jinan, Shandong Province in Jan 15, 2009 and discuss problems with the diagnostic process. Methods The patient＇s respirato- ry tract samples and throat swabs were tested with regard to nucleic acids using RT-PCR and real-time RT-PCR. Results The results of nucleic acid detection of the second throat swab were positive according to RT-PCR with the specific HSNA primer; the results of nucleic acid detection of all respiratory tract samples were positive according to RT-PCR and real- time RT-PCR with three specific primers （M, HSHA and HSNA primers）. Conclusion The case was confirmed to be a human case of Avian Influenza A （HSN1） virus infection. Respiratory tract samples were appropriate samples for the nucleic acid detection of the avian influenza virus.
