摘要
目的:探讨Wallis系统用于腰椎退行性疾病的手术方法及疗效。方法:自2007年11月~2008年8月采用Wal-lis系统治疗腰椎退行性疾病30例共36个节段,其中腰椎间盘突出症伴节段不稳21例、腰椎管狭窄症9例。并分别于术后2周、6个月进行随访,依照JOA评分及手术节段相邻上下椎间隙高度对其疗效进行评价。结果:本组30例均获得随访。根据JOA评分方法,术后平均改善率2周时为72.3%、6个月时为73.5%;手术节段相邻上下椎间隙高度与术前比较差异无统计学意义,术后2周、6个月椎间隙高度比较差异无统计学意义。结论:腰椎棘突间动态稳定系统(Wallis)治疗符合其适应症的腰椎退行性疾病,临床疗效良好,操作简单,手术时间短,损伤小,出血少且可避免融合技术引起的不良后果。
Objective:To explore the application and effect of posterior dynamic stabilization system (Wallis) in lumbar degenerative disorders. Methods: From November 2007 to August 2008, 30 patients with 36 segments of lumbar degenerative conditions were treated by Wallis, including 21 cases of lumbar disc herniation with degenerative instability of the segments and 9 cases of lumbar spinal stenosis. Follow-up Was done 2 weeks and 6 months respectively after the operation. The curative effect was evaluated by the scoring system proposed by Japanese Orthopedic Association (JOA) score and also the intervertebral height of adjacent segments. Results:All patients were followed up. According to JOA seoring system, recovery rate was 72. 3% 2 weeks after operation and 73.5% 6 months after operation. The intervertebral height of adiacent segments 2 weeks and 6 months after operation were insignificantly different compared to the original, and no significant differences were found in intervertebral height. Conclusion: Posterior dynamic stabilization system (Wallis) in lumbar degenerative conditions of adaptive symptom can yield satisfactory clinical results, with easy operation, shortened surgical time, mini-invasion, less blood loss and avoidance of complications caused by fusion technology.
