经皮穿刺颈椎间盘摘除术的临床应用

The clinical application of percutaneous cervical discectomy

目的评价经皮穿刺颈椎间盘摘除术(percutaneous cervical discectomy,PCD)的临床应用疗效及价值。方法应用PCD技术对21例临床确诊颈椎间盘突出症患者的25个间盘,在C型臂下准确定位,局麻后行小切口约0.3cm路径穿刺,正侧无误,冲击扩张后置工作套管,再用髓核钳反复夹取突出髓核组织,残留部分用切割器切吸。结果21例患者均获得随访,平均10个月(1-18月)。疗效评估结果,优11例(52.3%),良5例(23.8%),可4例(19.1%),差1例(4.8%)。总有效率95.2%,优良率76.1%。结论经皮穿刺颈椎间盘摘除术能准确有效解除颈椎间盘突出症的症状和体征,具有创伤小,恢复快,并发症少,疗程短等特点。

Objective To evaluate the curative effect and value of percussion cervical discectomy（PCD）.Methods By PCD technology, we treat 25 discs in 21 cases of cervical disc disease. Under the C-arm positioning accuracy,we take small incision about 0.3cm after local anesthesia,insert the rear casing after impacting and expanding, and then repeatedly clamp folder nucleus from prominent organizations,Some residual truncated by cutter.Resulta 21 patients were followed up on the average of 10 months（1 -18months）.The result indicated that the case of superior efficacy were 1 1（52.3%）,good efficacy were 5（23.8%）,general efficacy were 4（19.1%） and bad efficacy were l（4.8%）.The effective rate was 95.2% and the good rate was 76.1%.Conclusions Percussion cervical discectomy can accurately and efficiently relieve the symptom and sign of cervical disc disease,with trauma and rapid recovery,less complication,and shorter course of treatment.
