进气流速及呼气末正压对机械通气患者呼吸力学的影响 被引量:2

摘要 目的观察进气流速及呼气末正压(PEEP)对机械通气患者呼吸力学影响。方法11例无明显心肺疾病的机械通气患者随机交替变换20,40,60L/min3种进气流速及0,5,10cmH2OPEEP,测定呼吸力学、循环及动脉血气结果并进行分析。结果进气流速及PEEP增高可致进气峰压增高并有叠加效应(P<0.01);PEEP增高,呼气平台及平均气道压随之增高(P<0.05),进气流速对后者有间接影响;在一定范围内,进气流速及PEEP会影响呼吸系统顺应性,而呼吸阻力主要受进气流速影响(P<0.05);各实验组间循环及动脉血气分析比较,差异无显著性,未发现进气流速和PEEP对各观察值有交互作用。结论进气流速及PEEP改变对机械通气患者的呼吸力学、造成不同的程度影响,但两者无交互作用。
