肱骨外髁解剖钢板治疗肱骨髁间粉碎性骨折11例

11 cases of humeral intercondylar comminuted fractures treated with anatomical plate

目的总结采用鹰嘴截骨+肱骨外髁解剖钢板内固定治疗肱骨髁间粉碎骨折的疗效。方法自2006年6月至2008年8月,采用鹰嘴截骨+肱骨外髁解剖钢板内固定治疗肱骨髁间粉碎性骨折11例。结果术后随访时间4～15个月,平均8个月。根据Cassebaum评分标准,其中优4例,良5例,可2例,优良率为81.6%。结论采用鹰嘴截骨+肱骨外髁解剖钢板内固定治疗肱骨髁间粉碎性骨折,具有复位固定牢靠,早期功能锻炼有助于关节功能恢复,为现阶段治疗肱骨髁间骨折的有效方法。

Objective To summary the surgical management and its results of humeral intercondylar comminuted fractures through approach of osteotomy of olecranon （AOO） with humeral lateral condyle anatomical plate. Methods From June 2006 to Augest 2008,11 patients of intercondylar fracture of humerus, 7 males and 4 females with a mean age of 46.1 years（range 25 - 70 years） , were treated surgically through AOO, and the fracture was reduced and fixed with humeral lateral condyle anatomical plate. The patients were 7males and 4 females with a mean age of 46.1 years （ range 25 - 70 years）. Results The patients were followed up from 4 months to 15 months,8 months on average. According to Cassebaum Rating System, there were 4cases rated as excellent,5 good,2 fair, and one poor. The excellent and good rate was 81.6%. Conclusion The technique of through approach of osteotomy of olecranon （AOO） with humeral lateral condyle anatomical plate offers many advantages, such as sufficient exposure easy to get anatomical reduction, stable fixation and earlier exercise, which may be a promising way for the treatment of intercondylar fracture of humerus at present.
