可控式眼内硅油吸引器的制作及临床应用

Controllable intraocular silicone oil suction unit and its clinical use

目的：探讨眼内硅油取出的有效方法，研究视网膜复位眼的硅油取出手术效果。方法：作者研制了一种可控式眼内硅油取出装置，并应用于４３例４４只眼内硅油取出手术。结果：本装置由吸引针头、手柄、引流管和负压源四部分组成；采用普通持续负压源作为吸引动力，通过一个手指按压手柄上的进气孔来控制全部吸引过程。术中在６～１５分钟（平均８分钟）内将硅油整体吸出。术后平均随访５．３３个月，视力提高者２４只眼（５４．５５％），不变１１只眼（２５％）；３只眼发生低眼压（＜１０ｍｍＨｇ），３只眼复发视网膜脱离。未发生与该装置有关的并发症。结论：可控式眼内硅油吸引装置具有简单、安全、高效的特点；临床应用的经验值得推广。

To explore an effective method for intraocular silicone oil removal and study the outcome of silicone oil removal surgery in eyes with attached retina,we developed a controllable intraocular silicone oil suction unit(CISOSU),which was used in 44 eyes of 43 patients.The CISOSU consisted of suction needle,handpiece,drainage tube,and vacuum source.The suction force was produced by a common persistent aspirator,and the suction process was controlled with a finger by covering the ventilatory hole on the handpiece.In surgery,silicone oil(5000～5700cSt) was removed in its entirety within 6～15(mean:8) minutes.During a mean follow up of 5.33 months postoperatively,visual acuity was improved in 24 eyes(54 55%),unchanged in 11 eyes(25%);hypotony(IOP<10mmHg) was present in 3 eyes;retinal redetachment in 3 eyes.No operative complications relative to the use of this CISOSU occurred.We found that CISOSU was simple,safe,and effective.The outcome of silicone oil removal in eyes with attached retina was satisfying.Therefore,CISOSU deserved to be popularized.
