三叉神经痛显微血管减压术的远期疗效观察

Long term efficacy of microvascular decompression for trigeminal neuralgia

目的：评价随访１年以上的三叉神经痛显微血管减压患者９１例的远期疗效。方法：随访日本山形大学医学部１９８２～１９９２年间６１例，宁夏医学院１９９１～１９９６年间３０例患者，时间１３～１２６月（平均６０月）。结果：术后早期治愈率为９３．４％，随访得到患者主观满意率８０％，术后疼痛未缓解率是６．６％，复发疼痛率为１５％，术后并发症发生率７．７％。结论：采用显微外科技术重视三叉神经根附近痛因的解除对提高手术远期疗效是重要的。

Objective: To evaluate the long term follow up efficacy and subjective satisfaction of microvascular decompression in 91 patients with trigeminal neuralgia who were followed up more than 1 year. Methods: Patients comprised 61 cases who underwent MVD between 1982 and 1992 in Yamagata University and 30 cases who underwent MVD between 1991 and 1996 in Ningxia Medical College. They were followed up from 13 to 126 months (mean 60 months), and follow up results were studied. Results: The immediate postoperative cure rate were 93 4%. Satisfactary results obtained by follow up were 80%. Incomplate pain relief were 6 6%. Recurrence pain rates were 15%. Occurred rates of postoperative hearing and facial sensory dysfunction were 7 7%. Conclusion: Improving immediate postoperative cure rate and decreasing complication ofter MVD by using improved techniques of MVD are important and feasible obtain satistfactory follow up results.
