脊膜瘤MRI表现及其病理基础相关性研究 被引量:2

摘要 目的:探讨脊膜瘤MRI表现特征与病理关系。材料与方法:搜集行MRI扫描且有手术病理结果的脊膜瘤19例,观察肿瘤部位、数目、瘤体信号、肿瘤与脊髓交界带信号及硬脊膜改变,与手术病理结果对照并作统计学处理。结果:肿瘤单发18例,多发1例。髓外硬脊膜下间隙18例,硬脊膜外间隙1例。瘤体T1WI和T2WI信号强度与肿瘤组织学类型无关(P>0.05)。瘤体表现为混杂信号者8例,钙化是肿瘤信号不均的主要原因。全部病例均未见囊变及出血。瘤髓交界带信号表现与有无粘连有关。肿瘤邻近硬脊膜局限性增厚1例。结论:脊膜瘤多数位于硬脊膜下间隙,少数位于硬脊膜外间隙;单发多见,可以多发,且无囊变及出血。脊膜瘤MRI表现难以推测肿瘤组织学类型。瘤髓交界带T2WI呈低信号时,肿瘤与脊髓可能存在粘连。脊膜瘤可引起硬脊膜增厚。 Objective: To study the MRI features of spinal meningiomas and its pathologic relationship. Materials and Methods: MRI manifestation of 19 cases of spinal meningiomas confirmed surgically and pathologiclly were retrospectively reviewed and analysed; in regard to distribution,number. signal intensity of the tumor and the strip between the spinal cord and the tumor (SCT), and the change of the dura. Results: 18 cases were located in intradural- extramedullary space, 1 case in extradural space. In 18 cases the tumor was single, 1 case multiple. The signal intensity of the trmor in T_1WI and T_2WI have no relation with the subtype of the tumor (P > 0.05). 8 tumors were mixed signal intensity, the heterogenous signal intensity of the trmors is mainly caused by calcification. The cystic change and hemorrhage of the tumors were not found in our cases. The signal intensity of the SCT have relationship with the presence or absence of adhesion. Meningioma caused the drua thickening in 1 case. Conclusion: The most of the spinal meningiomas were single, some cases were multiple. The most of the spinal meningiomas located in intradural-extramedullary space, but some in extradural space. The subtype of the spinal meningiomas can' t be deduced from the MRI manifestions of the tumor. Adhesion may be existed when SCT appear hypointense in T_2WI. The spinal meningiomas can cause the dura thickening.
