摘要 目的探讨凋亡抑制因子Survivin及凋亡促进因子Smac在食管鳞癌中的表达及临床意义,并分析二者的相关性。方法RT-PCR法检测20例食管鳞癌及20例癌旁正常组织中Survivin及Smac基因的表达情况;免疫组化S-P法检测53例食管鳞癌及53例癌旁正常组织中Survivin及Smac蛋白的表达情况,分析其与食管鳞癌患者各临床病理指标的关系及两者表达的相关性。结果食管鳞癌组织中Survivin基因表达水平(70.0%)明显高于癌旁正常组织(0%),Survivin蛋白在食管鳞癌组织中的阳性表达率为62.3%,而在正常食管组织无表达,免疫组化结果与RT-PCR结果一致;Survivin蛋白在食管鳞癌组织中的阳性表达率与浸润深度、分化程度及淋巴结转移均无相关性(P>0.05)。RT-PCR结果显示,Smac基因在食管鳞癌组织中的阳性表达率为40%(8/20),明显低于正常食管组织100%(20/20),此结果得到免疫组化的证实,Smac蛋白在食管鳞癌组织中的阳性表达率为41.5%,明显低于正常食管组织100%,而且Smac蛋白在食管鳞癌组织中的阳性表达率与组织分化程度、浸润深度及淋巴结转移密切相关(P<0.05)。Survivin蛋白与Smac在食管鳞癌组织中的表达呈明显负相关(P<0.05)。结论抗凋亡蛋白Survivin和促凋亡蛋白Smac之间存在着相互拮抗,相互调节的关系,在食管鳞癌细胞凋亡过程中共同发挥作用。 Objective To detect the expressions of Survivin and Smac mRNA and protein in esophageal squamous cell carcinoma,to investigate the clinical significance and analyze their correlation.Methods The Survivin and Smac mRNA expression levels in samples from twenty esophageal squamous carcinomas and their matched normal esophageal tissues were quantified by semi-quantitiative RT-PCR,and immunohistochemical staining(S-P method) was used to detect Survivin and Smac protein expressions in 53 cases with esophageal carcinoma and 53 cases with normal esophageal mucoma.To analyze clinical data and the relationship between them. Results The up-regulation of Survivin gene was observed in 70.0%(14/20)esophageal carcinomas in comparison with the paired normal esophageal tissues.Positive staining rate for Survivin was 62.3% in esophageal carcinoma tissues,while no positive staining in normal mucosa was(P〈0.05),and there were no correlation between Survivin positive expressions in esophageal carcinoma tissues and invasion depth,lymph node metastasis(P〉0.05).Down-regulation of Smac gene was quantified found in esophageal carcinomas(40%) in comparison with the paired normal esophageal tissues(100%).Positive staining rate for Smac was only 41.5% in esophageal carcinoma tissues,while 100.0% in normal mucosa(P〈0.05),the expression level of Smac was related to invasion depth,lymph node metastasis and differentiation(P〈0.05).Negative relationship was found between Survivin and Smac in esophageal carcinoma tissues(P〈0.05).Conclusion Survivin expressions are significantly correlated with Smac expressions,and may play an important role in the apoptosis in esophageal carcinoma together.
