摘要
目的:了解某三甲医院系统性红斑狼疮住院患者用药情况,探讨临床药师如何在临床路径中发挥作用。方法:抽取该院2008年第3季度76例系统性红斑狼疮住院患者病历做回顾性调查,将所得到的信息根据统计学方法进行归纳、整理,分析其总量、构成比、DDDs值、DUI值,并将其与该院全年数据或者国内外数据进行比较。结果:76例患者平均年龄34.7岁,男性3例,女性73例,平均疗程为12.9天,平均住院金额为5 062.76元。患者按泼尼松计平均用量为84.0 mg.d-1,属于大剂量范畴;羟氯喹的使用量最多;吗替麦考酚酯(骁悉)所占费用比例最高;抗菌药物使用率为47.40%。结论:该季度系统性红斑狼疮住院患者平均费用低于2008年该院平均住院患者住院费用,疗程比2008年住院患者平均住院疗程长;抗菌药物使用比率较为合理,药物费用构成比较小,但是使用合理性还需加强。
OBJECTIVE: To probe into the utilization of drugs for inpatients with systemic lupus erythematosus iSLE) in a three - A grade hospital and explore the approach for clinical pharmacists to play a role in clinical practice. METHODS: A retrospective study was performed on 76 SLE patients collected in a 3 - A grade hospital during the third quarter of 2008, and the data obtained were analyzed statistically with regard to the consumption quantity, constituent ratio, DDDs value and DUI value, then the data were compared with the annual data in this hospital and the data both at home and abroad. RESULTS: The mean age of the 76 patients were 34.7 years (3 males and 73 females) with an average course of treatment of 12.9 days and mean hospitalization expense of 5 062.76 yuan. The average dosage of prednisone (84.0 mg·d^-1) was in a high - dose range. Hydroxychloroquine took the lead in terms of consumption quantity while CellCept took the lead in terms of consumption sum. The proportion of patients who received antibiotics was 47.40% . CONCLUSION: The average cost for the SLE patients in the third quarter was lower than the mean cost for the inpatients in this hospital in 2008, and the average course of treatment of the SLE patients in the third quarter was longer than the average course of treatment of the inpatients in this hospital in 2008. The utilization of antimicrobial drug was reasonable but the rationality remains to be improved further, and the constituent ratio of the drug cost is on the lower side.
