超声用于瘢痕子宫中孕引产可视观察产程的效果分析

Ultrasound for scar in the pregnant uterus induced visual observation of the effect of birth process analysis

目的:观察阴道B型超声用于瘢痕子宫中孕引产可视观察产程的效果。方法:选择2008年1月～2009年6月行瘢痕子宫中孕引产术的120例产妇,按孕周分为15～18周的41例(A组)、19～21周的42例(B组)、22～24周的37例(C组),采用阴道B型超声在引产术前宫缩发动时、第一产程开始时观察宫颈成熟度;选择各组中宫缩发动时出现不规则宫缩的65例(A组23例、B组22例、C组20例)产妇,肌注哌替啶,观察术前、肌注前、肌注后的宫颈成熟度。结果:中孕产妇宫颈长度和宫颈管宽度随着孕周的增加而缩短和增宽,但变化不明显,P>0.05;宫缩发动时到第一产程开始时,变化显著,经统计学处理,P<0.05。中孕引产出现不规则宫缩时,应用哌替啶肌注调节宫缩,肌注前后测量宫颈长度和宫颈宽度,进行比较,经统计学处理,P<0.05。结论:阴道B型超声用于瘢痕子宫中孕引产可视观察产程,了解掌握宫颈成熟度,发现不规则宫缩及时调节,有利于避免并发症的发生,效果可靠,值得临床推广。

Objective：To observe the effect of the vaginal B-mode ultrasound for trimester pregnancy of the scarred uterus. Methods＇Selected 120 parturients who were made the operation of scarred uterus trimester pregnancy from January 2008 to June 2009.According to the gestational age,the 120 parturients were divided into 41 cases （A group）of 15-18 weeks, 42 cases（B group）of 19-21 weeks and 37 cases（C group）of 22-24 weeks.Used the B-mode ultrasound to observe the cervical ripeness in the preoperative induced contraction initiated and the start of the first stage of labor.Chose the 65 cases（23 cas- es of A group,22 cases of B group,20 cases of C group）when occurred irregular uterine contraction,intramuscular the pethidine and observed the cervical ripeness before operation,before intramuscular and after intramuscular.Results：The cervical length and cervical canal width shorten and broaden with the gestational age,but the change was not obvious,P〉0.05;the change was significant from the uterine contraction to the first stage of labor, P〈0.05.Used the pethidine when found irregular uterine contraction,measured the cervical length and width before and after intramuscular,treated statistically,P〈0.05. Conclusion：Using the B-mode ultrasound to observe the trimester pregnancy of the scarred uterus,to investigate and master the cervical ripeness,to regulate timely when find irregular uterine contraction will help to avoid complications,the effect is reliable and is worthy to be popularized.
