摘要
目的:探讨白细胞介素-6(IL-6)基因启动子-572 C/G多态性各等位基因及基因型在2型糖尿病肾病患者中的分布频率,并分析其基因型与2型糖尿病肾病的相关性。方法:采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)技术检测183例2型糖尿病肾病患者(其中91例伴糖尿病肾病)及105例正常对照组IL-6基因多态性,包括IL-6基因启动子-572C/G位点,同时采用酶联免疫吸附试验(ELISA)检测血清IL-6水平。结果:糖尿病肾病组IL-6血清水平显著高于糖尿病不伴肾病组和对照组(P〈0.01),IL-6基因多态性在两组人群中的分布差异存在显著性差异(P〈0.05),等位基因频率的相对风险分析发现,G等位基因携带者患糖尿病肾病的风险是C等位基因的1.618倍(OR=1.618,95%CI:1.082~2.418),携带G等位基因的糖尿病肾病患者IL-6血清水平显著高于不携带者(361.71±93.42pg/ml VS 343.03±86.54 pg/ml,P〈0.05)。结论:IL-6基因启动子-572 C/G多态性与糖尿病肾病的发病具有相关性,其中G等位基因可能是糖尿病肾病发病的遗传易感基因,携带G等位基因的个体可能通过促进IL-6的高度表达进而增加2型糖尿病肾病的发病风险。
Objective: To study the relationship the allele frequencies and genotype distribution of interleukin(IL-6) gene promoter -572C/G polymorphism in Chinese patients with type 2 diabetes mellitus and diabetic nephropathy (DN),And analyze association the serum levels and genotype of IL-6 with type 2 diabetic nephropathy. Methods: The IL-6 gene promoter --572C/G polymorphism were detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP)analysis among 183 patients with type 2 diabetes mellitus and 105 healthy controls. The serum level of IL-6 was determined by enzyme-linked immunosorbent assay(ELISA). Results: The diabetic nephropathy group showed significantly higher serum levels of IL-6 than that of control group (P〈0.01). There was significant difference in frequencies of allele and genotype in --572C/G polymorphism between type 2 diabetic nephropathy group and control groups, respectively. The relative risk suffered from type 2 diabetic nephropathy of Gallele was 1. 618 times of the C allele (OR=1. 618,95%CI: 1. 082-2. 418), the serum IL-6 level was significantly higher among carriers of G allele as compared with non-carriers (C allele)(361.71± 93.42 pg/ml VS 343. 03± 86.54 pg/ml, P〈0. 05). Conclusion: IL-6 gene promoter -572C/G polymorphism was associated with type 2 diabetic nephropathy, and G allele may be a genetic risk factor of type 2 diabetic nephropathy. In which the IL- 6 G allele carriers may be increase risk by enhancing the IL-6 expression in the pathogenesis of type 2 diabetic nephropathy.
