博来霉素联合白细胞介素-2治疗恶性胸腔积液的研究

The application of bleomycin combined with Interleukin-2 in the treatment of malignant pleural effusion

目的：观察博来霉素联合白介素-2胸腔灌注与单用顺铂治疗恶性胸腔积液的有效性和安全性。方法：56例恶性胸腔积液患者的治疗用博来霉素联合白介素-2（A组）30例,单用顺铂（B组）26例。用一次性中心静脉导管行胸腔置管和闭式引流胸液,A组胸腔给药：博来霉素45~60 mg＋白介素-2（IL-2）100万U＋注射用生理盐水50ml胸腔内注射 B组胸腔给药：顺铂60mg/m2＋生理盐水50ml。结果：A组、B组治疗总有效率分别为88.67%和57.7%,差异有统计学意义（P〈0.05）。A组发热及胸部不适发生率较B组高,B组胃肠道副反应高于A组,但均无统计学意义（P〉0.05）。结论：胸腔闭式引流后灌注博来霉素联合白细胞介素-2治疗恶性胸腔积液的疗效优于单用顺铂组,且毒副反应轻微。

Objective： To evaluate the efficacy and safety of bleomycin combined with Interleukin-2 and single ciso diaminodichroplatin for malignant pleural effusion. Methods ： 56 individuals of malignant pleural effusion were divided into two groups： bleomycin combined with Interleukin-2 group （A group）including 30 individuals; single cisdiaminodichroplatin group（B group） including 26 individuals. Malignant pleural effusions were excreted cleanly from indwelling chest tube. A group was injected by thorax with 40-60mg of bleomycin and 1000000 units of Interleukin- 2, B group was injected with 60-80mg cis-diaminodichroplatin. Results： The total efficiency of A group and B group were 88. 67% and 57.7%, there was significant difference （P〈0. 05）. The rate of fever and chest discomfort in A group was higher than B group, the rate of gastraintestinal reactivity of B group was higher than A group,without significiant difference （P〉0. 05）. Conclusion：Bleomycin combined with Interleukin-2 were efficient and safety in the treatment of patients with mass malignant pleural effusions. It deserves further applications.
