GlideScope视频喉镜在预计困难气道中的应用观察 被引量:4

摘要 【目的】观察GlideScope视频喉镜在处理预计困难气道全麻气管插管时对喉部的显露时间、插管所用时间及血流动力学的影响。【方法】40例择期手术准备行全麻气管插管的成年病人,随机分为G组和P组,每组20例,术前处理和麻醉诱导相同。G组采用GlideScope视频喉镜进行气管插管,P组则采用Shucman普通直接喉镜进行气管插管。记录两组喉部暴露时间、喉部暴露Cormack—Lehane分级和插管时间,并分别在诱导前(T1)、插管前(T2)、插管后即刻(T3)及插管后1min(T4)、3min(T5)、5min(T6)六个时刻记录患者收缩压(SBP)、舒张压(DBP)、心率(HR)。【结果】①喉部暴露Cormaek Lehane分级:G组患者Ⅰ级暴露者19例(95%),Ⅱ级暴露者1例(5%),P组I级2例(10%),Ⅱ级10例(50%),Ⅲ级8例(40%)。此8例中Mallampatis试验Ⅲ级者5例,颏甲间距异常3例,组间有显著差异(P〈0.05);②G组平均插管时间较P组短[(64.7±13.38)sVS(186.2±143.09)s],有显著性差异(P〈0.05);③使用GlideScope视频喉镜可减轻气管插管的应激反应。【结论】GlideScope视频喉镜预计困难气道的气管插管中明显优于普通喉镜。
