腹腔镜肠粘连松解术21例报告

Laparoscopic intestinal adhesiolysis:with a report of 21 cases

目的:探讨腹腔镜肠粘连松解术的效果。方法:回顾分析我院应用腹腔镜行肠粘连松解术治疗肠粘连和(或)粘连性肠梗阻21例患者的临床资料。结果:全部病例均顺利完成手术,无中转开腹,2例术中小肠破裂,经镜下缝合修补,术后均无肠漏等并发症发生;平均手术时间55min,术中平均出血20ml,术后平均住院4.6d;术后随访6～45个月,均无粘连性肠梗阻和顽固性腹痛复发。结论:腹腔镜肠粘连松解术具有患者创伤小,康复快,并发症少,疗效确切等优点,能较大限度地减少术后腹腔内再粘连,只要选择好适应证均可取得良好的治疗效果。

Objective：To explore the effect of laparoseopic intestinal adhesiolysis. Methods：Eaparoscopic adhesiolysis was applied in 21 cases of adhesive intestinal obstruction or intestinal adhesion. Results ： All of patients were treated by laparoscopy successfully, and none of them were converted into open surgery. Intestinal disruption occurred during operation in 2 patients and was mended successfully by laparoscnpie suture. No postoperative complications, such as intestinal leakage, occurred. The average operation time was 55min;the average intraoperative blood loss was 20ml ; the average postoperative hospital stay was 4.6d. No relapse symptom was found such as adhesive intestinal obstruction or refractory abdominal pain in the follow-up of 6-45 months. Conclusions：Laparoseopic adhesiolysis has advantages of less tissue damage, short recovery time,less complications, exact therapeutic effect and so on,and can reduce the incidence of post-operative abdominal re-adhesion. Satisfactory therapeutic effect can be achieved with strict indication.
