摘要 目的:探讨64层螺旋CT成像技术对小儿气管、支气管异物的显示效果及其在临床诊断中的应用。方法:对4例气管、支气管异物患儿行单层螺旋CT扫描,对17例气管、支气管异物患儿行64层螺旋CT扫描,后处理技术采用多平面重建(MPR)、曲面重建(CPR)、最小密度投影(MinP)及CT仿真内窥镜(CTVE)。结果:21例气管支气管异物位于主气管8例、右侧支气管9例、左侧支气管4例,另外2例通过间接征象诊断,内窥镜结果与CT诊断一致。结论:64层螺旋CT多平面重建(MPR)和曲面重建(CPR)、辅助仿真内窥镜(CTVE)成像技术能直观显示异物的位置、大小、形态、及与周围组织的关系;较单层螺旋CT图像质量及异物显示均有显著提高,是检查小儿气管异物及指导内窥镜摘取的最佳选择方法。 Objective: To explore the Comparison of 64-Slice Spiral CT in Clinical Diagnosis of Foreign Body in Trachea of Children; Methods: 4 cases of foreign body in trachea and bronchus of Children were examined with Single Slice Spiral CT scanning; 17 cases of foreign body in trachea and bronchus of children were examined with 64-Slice Spiral CT scanning; Posttreatment Technology includes Multiplanar Reconstruction (MPR), Curved Multi-planar Reconstructions (CMPR), Minimum Intensity Projection (MinP), CT virtual endoscopy (CTVE), which enable to make a definite diagnosis and foreign bodies were taken out under endoscopy. Results: the 21 cases of children's foreign bodies in trachea and bronchus containing 8cases of foreign bodies in the trachea, 9cases in the right bronchus, 4 cases in the left bronchus; through indirect sign diagnosis, the results of 2 cases achieved under endoscopy and CT diagnoses were of coherence. Conclusion: 64-Slice Spiral CT visually displays the location, size, quantity and the relation with surrounding tissue of foreign bodies. Comparing with picture quality and foreign body showing of Single Slice Spiral CT, especially, the Muhiplanar Reconstruction and Curved Multi-planar Reconstructions have been enhanced largely, which improves the accuracy of preoperative diagnosis and clearly instructs the clinical treatment.
