Nipple reconstruction with modified S-flap

目的:改良原有的S形皮瓣乳头再造法,减少再造乳头因瘢痕挛缩造成的变形,使再造乳头外形更为逼真。方法:在S形皮瓣法的基础上,将乳头顶端皮瓣的切口设计由圆弧形改为"S"形,在乳头基底处增加两个小三角瓣。各瓣对应分层缝合形成乳头状隆起。结果:共有手术病例16例,乳头再造17只。其中11例经过1～2年的随访,再造乳头外形较为理想。结论:由于手术切口的改良,减少了切口瘢痕的挛缩,从而使再造乳头的外形更加的完美。

Objective To demonstrate a modified S-flap nipple reconstruction technique which can reduce the recon structed nipple distortion that caused by scar contracture, and make the nipple look more natural. Methods On the basis of S-flap nipple reconstruction, we have modified the round incision to S-shape on top of the nipple flap with two small triangular flaps added to the fundus of the nipple. Each of the flaps was sutured correspondingly and a new nipple was elevated. Results 17 nipple reconstructions were performed for 16 patients. The results from the 11 postoperative follow-up cases have proved satisfactory to both patients and surgeons. Conclusion The enhanced incisions can reduce the scar contracture, therefore, the reconstructed nipple looks more naturally.
