小腿创伤的显微外科修复暨美学思考

Repair of leg wound with microsurgery and its' aesthetic thinking

目的:探讨小腿及足部创伤的显微外科修复方法。方法:回顾一年多来应用各种游离组织瓣修复小腿及足部皮肤软组织缺损、骨外露骨缺损创面,总结经验教训。结果:25例患者26个游离组织瓣成功移植25个,1个骨组织复合瓣移植失败,但创面全部得到一期修复。结论:受区血管选择、组织供区选择、手术设计操作及术后观察处理是小腿创面显微外科修复成功的四个关键环节;修复创面的同时亦要兼顾供区和小腿的功能与美观。

Objective To investigate the ways to repair wounds of leg and foot with microsurgery. Methods Many kinds of free flaps were used to repair wounds of legs and feet with extensive soft tissue defect and exposed or defected bone during late one year. Rsults 25 flaps of 26 in 25 patients survived well, one composite free flap with bone failed, but all wounds healed with one stage repair. Conclusion The selection of donor site and recipient site, precise operation, and careful postoperation monitoring are the four key points for free flap grafting, attention should be paid to keep functional and cosmetic part of flap donor sites and legs when the wound were repaired.
