摘要
目的:探讨儿童先天性白内障手术时依据视觉发育的特点如何选择合适度数的人工晶状体植入。方法:收集在2005/2008年于我院施行白内障人工晶状体手术的儿童患者136例178眼,手术时年龄分为两组:A组2~5岁70例102眼,B组6~14岁66例76眼,观察术后1wk~1a患儿的眼轴长度和屈光度变化。结果:A组平均眼轴长度变化为3.08±1.89mm,平均屈光度变化为4.12±2.92D,B组眼轴长度平均变化为0.84±0.76mm,平均屈光度变化为2.07±1.36D。结论:儿童人工晶状体眼仍存在视觉发育的近视趋向,故较小年龄患儿人工晶状体度数应以低度欠矫为原则。
AIM : To investigate how to select degrees of intraocular lens(IOL) according to vision developmental features in children with congenital cataract. METHODS: One hundred and seventy-eight eyes of 136 patients from 2005/2008 in our hospital underwent phakic IOL. The children were divided into two groups by age at the time of implantation: Group A(102 eyes of 70 cases, aged 2 to 5) and Group B(76 eyes of 66 cases, aged 6 to 14).The changes of axial length and refraction were observed. RESULTS: For Group A, the mean length change of axis oculi was 3.08± 1.89mm, and the mean change of diopter was 4.12 ± 2.92D. For Group B, the mean length change of axis oculi was 0.84±0.76mm, and the mean change of diopter was 2.07 ± 1.36D. CONCLUSION: Myopic shift exists in pediatric pseudophakic eyes, so the degrees of IOL of younger child should be based on the principle of a low degree of undercorrection.
