贫困精神病患者免费服药3年随访分析

The follow-up analysis on free medication policy for psychiatric patients in poverty in three years

目的:评估免费服药对社区贫困精神病患者的治疗效果。方法:对160例社区贫困精神病患者随访,比较免费服药治疗3年前后患者服药依从性、病情稳定性、社会功能性、住院频繁性、社会破坏性等的变化,分析实行免费服药治疗的疗效。结果:遵医服药者从免费治疗前57例提高到146例,增加60.9%;间断服药都从59例减少到12例,减少79.6%:不服药者从44例减少到2例,下降95.4%。病情的稳定好转率从28.8%提高到93.7%。社会功能得到改善,不能自理生活者从62例下降到19例,下降69.3%。再住院次数及攻击行为明显减少,未住院患者从34例增加到143例,增加76.2%;无冲动行为者从71例增加到130例,增加45.5%。结论:贫困精神病患者需要正规系统治疗,免费服药是患者的康复途径之一,提高了患者的生活质量,减轻了经济负担,获得了治安稳定的社会效益。

Objective： To evaluate the effect on free medication policy for patients with psychosis in poverty in the community. Methods ： The research included 160 patients with psychotic disorders in poverty level in the community and compared the changes such as therapy comphance, state of the illness, function of the society, frequency of hospitalization and social aggression before and after the free medication policy in three years, evaluating the effectiveness of the pohcy. Results： The study showed patients who followed doctor＇s prescription increased from 57 cases to 146 cases by 60.9% ; whereas patients who discontinued treatment decreased from 59 to 12 by 79.6%;and patients with treatment refusal decreased from 44 to 2 by 95. 4%. At the same time, improvement rate for the condition increased from 28.8% to 93.7%. Social function had been improved. No self- care patients decreased from 62 to 19 by 69.3 %. It significantly decreased for re- hospitalization frequency and aggressive behavior. Patients without hospitalization increased from 34 to 143 by 76.2%; patients without impulsive behavior increased from 71 to 130 by 45.4%. Conclusion： Psychotic patients need standard and systemic medication treatment in poverty. Free medication policy is one of rehabilitative way for patients with psychotic disorders, improv- ing the quality of life, relieving economic burden, gaining social profit for security stability.
