脑发育性静脉异常的MR诊断

摘要 目的:分析、总结脑发育性静脉异常(DVAs)的MR典型表现,探讨其静脉解剖基础。方法:回顾分析了14例脑发育性静脉异常的MR平扫及增强扫描图像,总结其典型MR表现,记录DVAs的部位、引流方向,并分析其静脉解剖。结果:14例共发现15个DVAs及16个引流静脉。5例位于皮层下,9例位于脑室旁或深部。位于额部3例,顶部4例,小脑半球7例。其中4个DVAs为深部引流,1个DVAs同时有深部及表浅引流,其余9个为表浅引流。结论:DVAs的头部和引流静脉有其典型的发生部位及MR表现,最常见于脑室旁和皮层下区域,可由T2WI像发现病变,初步做出诊断,并由增强T1WI像得到证实。 Objective: To analyze and present characteristic MR findings of developmental venous anomalies (DVAs), and investigate the venous anatomy. Methods: We reviewed the routine and contrast-enhanced MR examinations of 14 patients with DVA. The characteristic MR findings and site of the DVA and direction of draining veins were recorded. Resuits: Fifteen DVAs with 16 draining veins were located. 5 cases were subcortical, 9 were periventricular or deep. Three case was frontal, 4 were parietal and 7 were in the cerebellar hemisphere. The draining veins were deep in 4 DVA and superficial in 9, and 1 DVAs had both deep and superficial draining veins. Conclusion: The caputs and draining veins of DVAs occurred in typical locations, with the periventricular and subcortical regions being the most common locations T2-weighted MR images could find the DVAs first, but diagnosis could be made from contrast-enhanced T1-weighted MR images.
