2型糖尿病患者生长激素与骨代谢相关关系的临床研究

摘要 目的检测2型糖尿病患者的生长激素(GH)、胰岛素样生长因子-1(IGF-1)、骨钙素(BGP)、降钙素(CT)、腰椎2~4骨密度,并探讨2型糖尿病与非糖尿病、糖尿病病程、糖尿病患者年龄对IGF-1、骨代谢的影响及相关关系。方法对60名2型糖尿病患者和30名健康对照者的GH、IGF-1、BGP、CT、C-肽(C-P)、胰岛素(INS)、空腹血糖(FBG)、糖化血红蛋白(HbA-Ic)和股骨颈、华氏三角、腰椎2~4骨密度进行检测。结果(1)糖尿病组患者的GH、FBG、HbAIc均高于对照组,2组对比有显著性差异(P〈0.05),INS、C-P、IGF-1、CT和腰椎2~4、华氏三角胺骨颈骨密度均低于对照组,2组对比有显著性差异(P〈0.05)。(2)糖尿病患者病程〉6年组的IGF-1、CT、INS、腰椎2~4骨密度均低于病程〈5年组,2组对比有显著性差异(P〈0.05)。(3)男性糖尿病患者年龄〉61岁组的IGF-1、CT、INS、腰椎2~4骨密度均低于年龄〈60岁组患者,2组对比有显著性差异(P〈0.05)。(4)糖尿病组患者各观察指标相关性分析,IGF-1与FBG、HbAIc呈显著负相关(r=-0.264、 -0.276,P〈0.05),与INS、CT、腰椎24骨密度呈显著正相关(r=0.569、0.481、0.456,P〈0.05)。结论糖尿病组患者与对照组比较以及糖尿病患者随着病程的进展及年龄增长,IGF-1相应降低,使骨吸收大于骨形成,骨密度减低。
