急性白血病多药耐药基因表达与化疗的关系——附25例临床分析

Expression of multidrug resistance gene in acute leukemia and its relationship to chemotherapy

目的 :了解急性白血病多药耐药基因表达情况与临床化疗的关系。方法 :对 2 5例初治急性白血病病人采用逆转录 PCR技术检测多药耐药基因 (MDR1)。并对其阳性和阴性表达的 2组病人化疗效果进行分析。结果 :本组病例 MDR1阳性表达 6例 (2 4 %) ,以急性非淋巴细胞白血病为主 ,占 5例 ,这 6例 MDR1阳性的急性白血病患者 ,没有 1例经 1个疗程化疗即能完全缓解 ;MDR1阳性的急性白血病患者 ,完全缓解率为 2 / 6,MDR1阴性者完全缓解率为 84 %,P<0 .0 5。结论 :急性白血病患者MDR1阳性 ,则化疗效果较差 。

Objectivs:To investigate the relationship between the expression of the mutidrug resistance gene (MDR 1) in actue leukemia and chemotherapy.Methods:The reverse trancription polymerase chain reaction(RT-PCR)technique was used to determine MDR 1 in 25 cases with untreated acute leukemia. The effects of chemotherapy were analysed in both the MDR + 1 and MDR - 1 group. Results:6 cases had positive expression of MDR 1 gene (24%), amount of them acute non-lymphoblastic leukemia occupied 5 cases.No any one expressing MDR + 1 got complete remission after one course of chemotherapy.The complete remission rate in MDR - 1 patients(84.2%)was significantly higher than that of MDR + 1 ones(2/6)(P<0.05).Conclusions:the acute leukemia patients with MDR + 1 respond worse to chemotherapy than those with MDR - 1 and possess an unfavorable prognosis.
