血管腔内治疗外伤性颅底骨折颈内动脉假性动脉瘤初步体会

目的研究血管腔内治疗外伤性颈内动脉假性动脉瘤的方法和疗效。方法回顾6例外伤性颅底骨折颈内动脉假性动脉瘤,2例冠脉覆膜支架覆盖动脉瘤颈,2例使用可脱球囊闭塞瘤颈,同时闭塞载瘤动脉,2例合并颈内动脉海绵窦瘘球囊封堵口并闭塞颈内动脉后使用电解可脱性弹簧圈填塞。结果4例一次性闭塞颈内动脉,2例冠脉覆膜支架覆盖动脉瘤颈,保持了颈内动脉通畅,旷置了假性动脉瘤。结论血管腔内治疗外伤性颅底骨折颈内动脉假性动脉瘤可使用多种方法及材料,微创,风险小、安全、有效。

Objective Study Endovascular treatment of traumatic carotid artery pseudoaneurysm of the method and effective. Methods Recalling the 6 cases of traumatic skull base fracture internal carotid artery pseudoaneurysm, two cases of coronary stent - graft coverage of carotid artery aneurysm, two cases of the use of balloon occlusion of aneurysms with detachable neck,at the same time the parent artery occlusion,2 cases of combined ball carotid- cavernous fistula（CCF） capsule sealing the mouth and after carotid artery occlusion using guglielmi detachable coil（GDC） packing. Results One - time four cases of internal carotid artery occlusion,2 cases of coronary aneurysm stent - graft coverage of the neck and to maintain patency of the internal carotid artery, exclusion of the pseudoaneurysm. Conclusions Endovascular treatment of traumatic fractures of the skull base carotid artery pseudoaneurysm can use a variety of methods and materials, minimally invasive,the risk of small, safe and effective.
