338例颈内中心静脉穿刺置管术并发症临床分析

Clinical analysis on complications of 338 internal jugular central venus catheterization cases

目的分析、探讨颈内中心静脉穿刺置管术并发症、发生原因和改进技术。方法对2001年1月至2009年5月338例应用颈内中心静脉穿刺置管术患者病历资料进行回顾性分析,研究相关的并发症和技术问题。结果338例颈内中心静脉穿刺置管术总并发症发生率为9.47%,其中局部渗血、血肿8例(2.37%),误穿动脉13例(3.85%),气胸2例(0.59%),局部红肿感染6例(1.78%),心律失常3例(0.89%)。结论颈内中心静脉穿刺置管术具有一定的风险,需从多方面、多环节分析原因,预防并发症的发生。

Objective To analysis and explore the complications of internal jugular central venus catheterization and the skills of improvement. Methods 338 patients underwent internal jugular central venus catheterization in our hospital from January 2001 to May 2009 were analyzed retrospectively to study correlative complications and technology. Results All kinds of complications oc curred in 32 cases（9.47% ）, including 8 cases of local hemorrhage and hematoma（2.37 % ）, 13 cases of puncturing the artery by mis- take（3.85 %）, 2 case of pneumothorax （0. 59%）, 6 cases of partial inflammation and infection （1. 78%）, 3 case of arrhythmia （0. 890%）. Conclusion Internal jugular central venus catheterization has acertain risk,so more attention should be paid to prevent from the occurrence of complications in various ways.
