摘要
目的评价动态三维超声对心脏瓣膜病诊断的意义。方法应用动态三维重建技术研究50例不同类型的心脏瓣膜病患者,其中男性25例,女性25例,年龄17~62岁。10例正常瓣膜者作对照。全部病例经心血管造影及/或手术证实。经食道超声检查并采集图像,通过计算机系统提取、分析及重建瓣膜,从不同的方向观察瓣膜的表面、形态及活动情况。结果在50例瓣膜病者中,5例为三尖瓣异常,29例为二尖瓣异常,16例为主动脉瓣异常。结论动态三维超声所反映的瓣膜整体性在二维超声显像中难以得到。
Objective\ To evaluate the significance of dynamic three dimensional echocardiography(3 D) in the diagnosis of cardiac valvular diseases(CVD).\ Methods\ 50 patients with different cardiac valvular diseases were studied by dynamic 3 D reconstruction technique.\ There were 25 men and 25 women(ages range from 17~62 years) in this study.\ The results were compared with 10 normal subjects.\ All patients were confirmed by catheter and/or surgery.\ Tomtec echo scan system was used for three dimensional reconstruction,and Hewlett Packard 2500 echocardiography(with 5.0/3.7 MHz multiplane transesophageal probe) was used for data acquisition.\ The valves were extracted,analyzed and reconstructed by computer system.\ We can view surface,shape and action of valve from different direction.\ Results\ There were 5 patients with tricuspid valvular abnormality, 29 patients with mitral valvular abnormality, 16 patients with aortic valvular abnormality in 50 patients with CVD.\ Conclusion\ The valvular entirety and quality acquired by dynamic 3 D could not be reflected in two dimensional echocardiography.\ It provided more accurate information for selected treatment method of CVD.
