肺部念珠菌病胸部CT表现

The Chest CT Representation of Lung Candidiasis

目的:探讨肺念珠菌病胸部CT表现。材料与方法:回顾性分析23例肺念珠菌病的胸部CT资料,并对照比较抗炎及抗真菌治疗后的胸部CT变化。结果:胸部CT主要表现为支气管周围炎症改变及肺内实变,其中支气管炎型9例(39.13%)、肺炎型12例(52.17%)、播散型2例(8.70%);其中形成空洞者7例(30.43%),伴有胸膜改变者3例(13.04%);抗炎后病灶无吸收10例(43.48%)、加重8例(34.78%)、出现新病灶5例(21.74%),抗真菌治疗后病灶变化则以吸收为主(20例,86.96%),二者差异明显(X2=72.74,P<0.01)。结论:CT表现能基本反映肺念珠菌感染的病理形态学特征,但CT征象缺乏特异性,早期诊断不易;动态观察治疗后的胸部CT变化,不仅能客观评价治疗效果,还可为其诊断提供有价值的参考依据。

Purpose： To discuss the chest CT representation of lung candidiasis. Materials and Methods： The chest CT information of 23 lung candidiasis was analyzed retrospectively, and the changes of chest CT were compared after the treatment anti - epiphyte. Results： The main chest CT representations were the inflammation change of surrounding bronehia and the permeability pathological changes of lung, in which 9 cases were bronchia inflammation（39. 13% ）, 12 were pneumonia （52. 17% ）, 2 were spread（8.70% ）, 7 formed cavity（30. 43% ） and 3 cases（ 13.04% ） attended by pleural change. There were 10 cases having no changes absorption after anti - inflammation treatment （43.48%）, 8 were aggravated （34. 78% ） and 5 new focuses appearing for 5 （21.74%）. Otherwise, the focus changes were mainly absorption after the treatment of anti - epiphyte （20patients, 86. 96% ）. There was significant difference between them （X2 = 72. 74, p 〈 0. 01 ）. Conclusion： The representation of CT could reflect the pathological morphological characteristics of lung moniliform germ infection, but they wer- en＇t specific, and they were difficult for making early diagnosis. To obsere the CT changes after treatment could not only could impersonally appraise the remedial effect, but could also provide the valuable referenced for the diagnosis.
