肝胆外科老年病人围手术期的肠外营养

Parenteral Nutrition Support to the Elderly Patients After Hepatobiliary Surgery

摘要 目的:探讨肝胆外科老年病人围手术期的肠外营养的合理作用。方法:21名肝胆外科术后老年病人随机分为实验组和对照组,实验组患者术后1~7d给予静脉营养,对照组患者术后常规补液。对两组患者术前术后进行指标测试。结果:实验组患者术后负氮平衡明显改善,肝、肾功能及免疫球蛋白水平与术前无明显差异,血浆蛋白的浓度得到维持。结论:肝胆外科老年病人术后围手术期的肠外营养治疗时,应注意低热量及合理的糖脂供热比,注意合并症采用“个体化”。 cases of the elderly patients after hepatobiliary surgery are divided randomly into parenteral nutrition group(PN) and control group. The patients of the parenteral nutrition group are supported with the early postoperative parenteral nutrition for 7 days, and the patients of the control group are given the routine intravenous infusion. The result shows that there are no significant dectrase of seroalbumin, hemoglobin and total protein but significant decrease of ALP and γ GT are discovered in PN group. TCL raise remarkably in PN group. On the contrary, there are significant increase of glucose but seroalbumin fall greatly in the control group. We can draw the conclusion that PN support to the elderly patients after hepatobiliary surgery should comply with the principle of hypocalorie and the ratios of calorie supplied by glucose and lipid should be 6∶4, which can partly correct negative nitrogen balance, maintain the level of albumin and improve immunofuction.
