岩斜区脑膜瘤影像学评估的临床意义 被引量:1

摘要 目的通过对岩斜区脑膜瘤(PCM)的影像学分析,评估手术风险,选择适当的治疗策略。方法回顾性比较59例PCM患者的术前影像学资料、手术入路的选择、手术切除程度、术后并发症等情况,分析术前影像学资料对制定肿瘤治疗方案的指导意义及对预后的评估。结果术前影像学检查包括MRI、MRA、MRV、CT、CTA、DSA等。MRI影像学提示伴海绵窦(CS)侵袭的32例,瘤周水肿明显的13例,包绕颅底大动脉及分支的25例,多颅窝生长的37例,肿瘤最长径≥5 cm的有14例,脑干明显受压19例。59例岩斜区肿瘤中,经颞—岩骨—乙状窦前入路35例,枕下乙状窦后入路14例,颞下及颞下经小脑幕入路7例,幕上下联合入路3例。手术全切肿瘤28例,大部分切除23,部分切除8例,死亡2例。颅神经损伤7例、脑脊液漏4例、皮下积液2例、颞叶水肿伴出血1例。结论术前详细的影像学分析可以个体化设计手术方案,减少手术并发症,判断预后。
