全麻诱导后留置导尿管对成年男性患者麻醉苏醒期血流动力学及不良反应的影响

Effect of Urinary Catheterization after the Inducement of General Anesthesia on Hemodynamics and Adverse Reaction in Male Patients in Anesthesia Recovery Period

目的:观察全麻诱导后留置导尿管对成年男性患者麻醉苏醒期血流动力学及躁动等不良反应的影响。方法:90例择期全麻下行胃癌根治术男性忠者分为两组,对照组(n=45)在清醒状态下行导尿术,实验组(n=45)在全麻诱导后行导尿术,观察两组患者麻醉苏醒期血压(BP)及心率(HR)的变化,以及苏醒期躁动程度和导管脱出率。结果:实验组患者气管导管拔除即刻、拔除后5min、10 min及15min时的SBP、DBP及HR均高于对照组(P<0.05).实验组患者躁动程度和导管脱出率也均高于对照组(P<0.01或P<0.05)。结论:成年男性患者全麻诱导质留置导尿管可导致患者苏醒期血流动力学指标急剧升高、以及躁动程度和导管脱出率的明显增加,导尿管留置宜在全麻诱导前实施。

Objective： To study the effect of urinary catheterization after the inducement Of general anesthesia on hemodynamics and adverse reaction in male patients in anesthesia recovery period. Methods ： Ninety male patients schedule for radical gastrectomy under general anesthesia were randomly divided into two group：experimental group（n=45）,the urinary catheter was inserted after the inducement of general anesthesia;control group（n=45）, the catheter was inserted before the anesthesia.The changes of blood pressure（BP） and heart mte（HR）,and the adverse reaction such as restlessness,prolapse of catheters in anesthesia recovery period were observed.Results：The systolic BP, diastolic BP and HR in experimental group was higher than that in control group at the time of removal of the tracheal robe,5,10 and 15 minute after removal （P〈0.05） .the degree of restlessness and the incidence of prolapse of catheters in experimental group was higher than that in control group （P〈0.01 or P〈0.05） .Concluslon：Urinary catheterization after the inducement of general anesthesia could heighten the hemodynamie index,increase the degree of restlessness and the incidence of prolapse of catheters in recovery period.It was advisable to insert the catheter before anesthesia induction.
